New Patient Forms

A comprehensive examination will most likely be performed the first time you visit. Even if you have had regular care under another dentist, we want to become familiar with your health. This will allow us to notice changes or problems more easily during future visits. We also usually clean your teeth, do an examination, and sometimes take X-rays. Then we will check the X-rays and your teeth for signs of decay, check your gums for change and check for signs of oral cancer or other diseases.

Another important part of every visit is updating your medical history. We will want to know if you've had any changes in your health or your medicines and obtain an overall picture of your health.

Appointments:

If you would like to make an appointment, please contact our office by phone or email. Our office facilitates communications and emergency calls, and appointments are available and welcome, especially for new patients seeking help.

Financial & Payments:

We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are check, cash, Visa, Mastercard, and Care CreditTM (APPLY NOW).

Cancellations:

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours' notice. We ask for this advance notice so that we can offer this appointment slot to another patient. Note that a fee may be charged if a patient does not show up for an appointment without sufficient notice.

For Your Visit:

On your first visit, we will need several forms completed in order to enter you into our system and properly bill your insurance. This will expedite the process and get you into seeing the Dr. right away. Please note our new patient forms are currently not available for completion on mobile phones or tablets. Please download and complete the forms below and bring with you to your dental appointment. Please note that the Notice of Privacy Practices and Dental Material Fact Sheet are to be viewed and do not need to be printed.

Also, prepare to have insurance information available and any types of payment such as cash, credit card, or personal check.
